Notice to Complainant:

1. By reporting this defect, you agree to return the defective sample (if requested and available) and that the defective sample has not been exposed or contains any body fluids, including blood or saliva, and that all safety precautions have been taken in order to transport the defective sample without any harm to those who will come into contact with the sample. (eg. removal of tubing, needles, IV Sets, etc…).  If the sample has been exposed, do not send the sample and speak with the CSL Representative concerning next steps.

2. Please note the form will only submit once all mandatory (*) fields are completed and the checkbox below is ticked. If you cannot complete all mandatory fields, please find the relevant "Product Complaint" details at https://www.csl.com/contact to report the complaint.

3. For complaints concerning clinical products, please do not use this form and refer directly to your manual.

Please retain this unit(s) in its exact form until advised by CSL. Please take photographs and attach to this form.

If we cannot contact you this might affect your ability to get a refund / replacement unit.
